Lost my passport along with my tourist VISA for the United States. Would I have to go through the entire tourist visa process again.

Hello everyone,

My girlfriend is Costa Rican and lost her passport along with her visa to the United States. Does anyone know if she would have to reapply for the visa or is it documented at the embassy. Any advice would be so helpful. I am so concerned that she will not be able to visit. )):
